Job Description

The Institute for Child and Family Health (ICFH) is a not-for-profit established in 1945 that provides behavioral health and prevention services to children and their families in Miami-Dade, Broward, and Palm Beach counties. It is one of the oldest and largest community mental health centers exclusively dedicated to the well-being of children and their families in the State of Florida. ICFH employs a staff of more than 400 full-time and part-time employees, and administers multiple contracts and grants from a diversity of funding sources in order to provide services in the following six areas: Child and Family Counseling; Substance Abuse Prevention; Early Childhood Programs; Violence Prevention; Literacy and Educational Development; and Professional Training Programs.

Job Summary:

Responsible for coordinating an array of mental health and substance abuse services through the wraparound process adapted for the Transition age young adults (18-21).  Responsible to ensure all Wraparound Phases are implemented to high fidelity.  Engage the young adult and family and orient them to Wraparound Team Approach. Complete strength, Needs and Cultural Discovery; complete Crisis stabilization plan, develop Wraparound plan, create and maintain the Wraparound team, link to other service systems (i.e. DJJ) and clinical supports (i.e. Medication management). Facilitate transitional and discharge process.


Duties include:

  • Responsible for scheduling and attending meetings to ensure coordination of care and effective communication within the team, to integrate the team in the wraparound process and to discuss client’s progress and compliance with services.
  • Serve as liaison between youth/families and neighborhood community agencies and service systems;
  • Help youth to identify their existing strengths and natural support network;
  • Develop a comprehensive assessment, wraparound plan, functional assessment and crisis plan for each client;
  • Coordinate and facilitate monthly team meetings;
  • Administer substance abuse testing as needed;
  • Complete and submit Incidental Funds requests as needs are identified by team;
  • Complete high quality documentation in accordance with agency policy and contract and submit within given deadlines.
  • Attend weekly individual supervision and weekly all staff team meetings;
  • Coordinate the discharge process and ensure required documentation is completed and forwarded to funder.


Targeted Case Manager Requirements:

  • Must have a Bachelor’s degree from an accredited college or university in social work, psychology or a closely related field.
  • Minimum of 1 year full-time equivalent case management experience serving the SED population and working with young adults (18-21 year olds.)
  • Must complete Wraparound 101, Motivational Interviewing, Natural Support and Engagement Training, within 3 months of hire and complete the Wraparound Coaching Process to meet certification.
  • Fluency in both English and Spanish is highly desired.
  • Must be able to wok flexible hours and willing to travel entire Miami-Dade County.
